Purpose of Training
Earlier this year, a Standard Recertification survey of our Certified Health and Home Care Agency (CHHA) was conducted by the NYS Department of Health between 9/24/18 to 9/27/18. The results of the survey identified areas in need of improvement. Consequently, we developed a Plan of Correction (POC) to address the need to ensure that all our staff are aware and up to date with regulations and our policies and procedures. Part of the POC is aimed at focusing on how to standardize patient assessment data, facilitate coordination of care across settings, and enable quality data documentation and measurement.
Over the months of November and December, Ms. Fran Arroyo delivered in-person training in Bronx, Brooklyn, Manhattan and Queens. Additionally, Ms. Arroyo has complemented her in-person trainings with this online version.
Completion of the training includes:
- Watching the video
- Completing and submitting the “Post Test”
Please note:
- To receive credit for the training you must watch the video until its conclusion, which will trigger the system to send you a “Completion” confirmation email
- You are required to forward the email you receive to your supervisor and Ms. Frances Arroyo to receive your test. Taking the test is also required.

Learning Objectives:
- Know all the deficiencies that were identified by the surveyors from NYSDOH
- Know the documentation/clinical tasks that will be required to be in compliance with the agency Plan of Correction (POC)
- Become familiar with the policies and procedures that will support the Plan of Correction (POC)
- Step 1) Locate the course you want to take
Step 2) On the course home page, select “Take This Course”

Step 4) Once logged in, complete the registration form, ensuring to fill out all the mandatory fields, then click “Submit”

Note: If you are using Chrome, you can clear you cache by clicking on the three dots in the upper-right corner of your browser and selecting “History.” Under “History”, on the left-hand side, select “Clear Browsing Data”. Set the “Time Range” as “All Time” and ensure that the box for “Cached Images and Files” is checked. Lastly, select “Clear Data”
